No recommendations were made by the Child Death Review Committee after investigations into three cases.
The committee reviews the deaths of children under the age of 19.
The first involved a four-year-old boy who died following complications of cerebral palsy.
The second was an 18-month-old girl who accidentally died in a house fire.
And the third case was a 17-year-old boy who died in a car crash.
In all three cases, the family or child was being assisted by the Department of Social Development.
